It is up to each individual climber to evaluate all the risks associated with climbing, including falling objects, avalanche, falling, etc., including the condition of any fixed protection, which may not be visible. Climbing is in fact very dangerous and people actually do die and get seriously injured routinely. Installation, monitoring and maintenance of fixed equipment is always the responsibility of the individual climbers.
